Image Release Form

Use of this form:  This form is to be used by the coach of the team that wishes to have an electronic image of their team, team members, or a team member posted on the Oneonta Soccer Club web page.

Instructions for use:  Each team member depicted in the electronic image must have their parent/ legal guardian sign this form.  If both parents/ legal guardians are not residing in the same household or have joint custody of the team member then both parents/ legal guardians must sign this form.

The below signed parents/ legal guardians give their permission to the Oneonta Soccer Club to use their childs image(s) for official publications including the Clubs website.  I understand that I/ we will not be compensated for my time or the rights to use the image(s). 

print team member's name,  sign parent/legal guardian's name, sign other parent/ legal guardian name if applicable
